Details for Aging Disability Resource Coordinator Position:

Job Duties for Aging Disability Resource Coordinator Position: 
  • Identify and maintain updated files of resources beneficial to the elderly and disabled in the region
  • Answer calls, provide information, refer consumers to appropriate resources or staff, and log all calls
  • Collect and enter client intake information, seek and link consumers with needed area resources
  • Mail requested information to consumers and prepare required reports
  • Maintain effective working relationships with caregivers, staff, and the public
  • Assist other departmental employees as needed and attend relevant conferences or trainings
  • Utilize standard office equipment and vehicle as necessary
  • Work effectively both independently and as part of a team, communicate and coordinate efforts, and meet deadlines
  • Follow all policies and procedures in place 
  • Completes any other related tasks as needed 
 
Qualifications and Desired Skills for Aging Disability Resource Coordinator Position: 
  • Bachelor’s degree in Social Work, Sociology or related field supplemented by one year related work experience preferred
  • Must have and maintain valid driver's license
  • Additional relevant work experience may substitute for education requirement on a year-for-year basis up to a maximum of two years 
  • RN with valid Kentucky nurse’s license preferred
  • LPN with valid Kentucky nurse’s license and four years' work experience in a related field preferred
  • If required by the funding agency, employee shall be licensed or certified
  • Ability to lift and push up to 25lbs
