Details for Clinical Rehabilitation Specialist position: 

  • $33.50-42 / Hour

  • Full-Time | Direct-Hire

Job duties for Clinical Rehabilitation Specialist

  • Deliver a wellness and recovery-focused service approach to all clients 

  • Foster collaboration within the SM FSP 1 team and maintain a client-centered, “whatever it takes” service model 

  • Conduct and facilitate training on best practices, including Assertive Community Treatment, co-occurring disorders, Dialectical Behavior Therapy, and Trauma-Informed Care 

  • Promote trauma-informed practices in all services and team activities 

  • Utilize Collaborative Communication Guidelines for clear, empathetic, and person-centered communication 

  • Train team members and clients on mental health and substance abuse prevention strategies within holistic wellness plans 

  • Participate in daily case reviews, assess potential clients for program eligibility, and collaborate with County Behavioral Health for assessments and referrals 

  • Provide psychotherapeutic counseling and rehabilitation services to help clients achieve social and personal goals.
    Document clinical activities, including diagnostic findings and treatment plans, and act as Care Coordinator for a client caseload 

  • Offer crisis counseling and evaluate clients for potential admissions to treatment facilities 

  • Engage with clients’ families to ensure understanding and cooperation in treatment plans 

  • Manage a caseload of 15-20 clients, coordinating services to ensure continuity of care 

  • Provide client transportation as needed based on daily schedules 

  • Collaborate with external agencies and practitioners 

  • Educate clients and families on diagnoses, treatment, and rehabilitation processes 

  • Assist clients in obtaining stable housing, healthcare, benefits, and essential life skills 

  • Participate in quality assurance and program evaluations 

  • Assist in the hiring and training of new staff 

  • Follow all policies and safety procedures in place 

  • Completes any other related tasks as needed

 

Qualifications and Desired Skills for Clinical Rehabilitation Specialist

  • Valid license as a Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) 

  • Individual must have BBS issued registration 

  • Must have valid driver's license (at least 2 years of experience) with reliable transportation 

  • Must comply with a TB assessment 

  • Must be able to successfully complete First Aid and CPR training 

  • Knowledge of assessment, diagnosis, counseling, treatment planning, human services, mental health trends, and relevant laws/regulations 

  • Experience working with individuals with severe mental illness, and with the chronically homeless or those at risk of homelessness 

  • Strong commitment to the rights of individuals with severe mental illness to live safely, engage in the community, and access support services 

  • Ability to perform effective counseling, develop cooperative relationships with clients and professionals, utilize community resources, and prepare clear reports 

  • Strong interpersonal, organizational, and time management skills, with effective oral and written communication abilities 

  • Ability to stoop, bend, squat, kneel, twist and reach
